Innovation in Health Care Award 2018
Alzheimer’s Los Angeles receives the SCAN Foundation’s 2018 Innovation in Health Care Award for the Dementia Cal MediConnect Project.
The Dementia Cal MediConnect Project is an initiative in California to improve healthcare for older, low-income adults with dementia and their family caregivers. It is a collaborative effort involving federal and state agencies, health plans, and Alzheimer’s organizations. Through this Project, health plans have changed their systems of care to make them more responsive to the needs of people with cognitive issues and their family caregivers, care managers have been trained in more dementia-capable care practices, and families have been provided with disease education and support.
The Innovation in Health Care Award ceremony took place in Sacramento on September 27, 2018 at the SCAN Foundation’s annual Long-Term Services and Supports Conference. Alzheimer’s Los Angeles was recognized alongside partner agencies and organizations.
